Emergency surgical embolectomy is recommended in hemodynamic unstable patients with massive pulmonary embolism in a facility with cardiac surgical capabilities . Catheter embolectomy should be performed in absence of cardiothoracic surgical backup . In our case, thrombolysis was contraindicated.
